Assessment Plan 

It is known that the assessment plan for the psychosocial model has been used to identify brief information about the patient’s history. Following that, the assessment pan for the psychosocial model has incorporated different components such as identification of the patient’s ID, medical history, medication list as well as information about the consumption of drug and alcohol, violence under physical as well as verbal torture, occupational as well as a family history of patents (Odum et al., 2020). Based on the selected components, the researcher has developed an assessment plan under the selected or given case study.

Importance of The Assessment Plan with The Selected Model

Based on a given case study, the research conductor has developed an assessment plan to proceed with the interview procedure. Throughout the whole interview procedure, the research conductor has seen that the patient has become restricted. Moreover, the selected patient also sometimes became agitated by the proceeding question. However, the lady is lacking from happiness under her relationship status as she told that she was brutally tortured by her ex-husband as well as from her current boyfriend. Hence, the lady is suffered through mental illness such as agitation, depression.

Intervention Plan

As per the context of the psychosocial model, it is known that the intervention plan is responsible for the delivery of best practices to the patients suffered through mental illness. Also, the doctors or psychiatrists, as well as social workers, first analyze the patient’s past as well as present situation (Duan et al., 2020). After that, they provide a better adaptation to promote health as well as the behavior of the sufferer. Following that, there are presences of basic elements such as patient’s autonomy, self-knowledge as well as self-help by the intervention plan for a psychosocial model (Snopkowski and Ziker, 2020). Also, this intervention plan is very beneficial for the patient to overcome the mental trauma that she gained during her relationships (Howe, 2002). Moreover, the intervention plan also benefits the patient to progress in her professional life which in turn, helps the patient to earn more money and achieve her dreams.

Evaluation of Psychosocial Factors in Disease and Treatment

By proceeding with the intervention plan under the psychosocial model, the research conductor has gained the knowledge to set future goals for the patient. These future goals are responsible for helping the patient to overcome her from mental trauma (Healy, 2005). Also, the research conductor has given its limelight on the positive aspects of the patient’s life (Engert, Grant and Strauss, 2020). This kind of activity shall be helpful or beneficial for the suffered patient to save her life from suicidal ideation.

Conclusion on Psychosocial Factors in Disease and Treatment
