Introduction

Self-management and support interventions are directly associated with improvement in the outcome for people with any long-term condition. Self-management steps are highly important, these allow one to have proper ideas and knowledge about the condition and also allow the patient to have unbiased opinions. Some of the common things which are included in self-management techniques are: providing patient-centered care and compassionate care. The whole team should be involved in the care process. Patients' visits should be planned and the major focus should be on care and prevention management (Chandio et al., 2022). In the essay, the issues which are faced by health services are discussed. Different issues which are discusses are, the first issue is inadequate access to basic healthcare services like a shortage of medical professionals, other is the problem of understanding the health technology The use of effective communication skills is discussed. Other than this self-management techniques need to be discussed and a sustainable model used by the students is also discussed.

Health services' challenges in meeting the demands of lifelong condition management

There are several issues faced by the health services when they work towards meeting the increased demand of the population living with some life-long condition. The first issue is inadequate access to basic healthcare services like a shortage of medical professionals and lack of any quality assurance. In addition to it, there might be inadequate spending on health, the research funding might also be insufficient. Another very important concern is if there is any insufficient administration of the financial allocation.

Another major issue is the increase in the cost of health care (Adu et al., 2019). Several studies state that it is becoming difficult for the common man to afford health care services and statistics state that about 40% of the people have medical debt. This hampers the care process and the patient suffers in the end. Since the cost of healthcare services is increasing, it will restrict several opportunities for people. People will not be able to afford even the most basic resources. 

Another important issue is the problem of understanding the health technology which is present. Technology plays an important role in the healthcare industry and it provides information about several departments. There is an integrated health service, in which information is being used. With the introduction of technology transparency and cost in the healthcare industry are improved (Zurynski et al., 2021).

Using Communication to Identify Problems, Set Goals, and Collaborate with Healthcare Workers Effectively

In the healthcare industry communication is a highly important skill and it is presented with several benefits. With communication the patients are encouraged to share information, in addition to it, patients can understand the situation in a better way. The patients become and develop more understanding of the technology. With efficient communication skills, people are motivated to take their medication at the right time. It also helps people to live a more healthy lifestyle. Skillful communication enables the healthcare provider so they establish a rapport with the patient. In addition to it all crucial healthcare information can be shared easily (Wang et al., 2023).

Collaboration between the healthcare workers is highly effective and it presents several advantages. The collaboration between nurses, physicians, and other healthcare members results in several advantages. It increases the team members' awareness, along with their skills and knowledge is also increased. This results in continuous improvement in the decision-making process. An effective team presents some important characteristics and these are respect, collaboration, and trust. Collaboration with professionals allows doctors to share their expertise with others. Other than this new technologies are also understood so the team can be led efficiently (Duda-Sikuła et al., 2023).

Person-centered Self-management Approach

The model which is chosen for the person-centered approach is the Stanford model. There are five phases of the model and these are: empathize, define, ideate, prototype, and test. The first stage is empathize, which is user-centric, in this empathic understanding should be present, to solve the problem. The physical environment should be assessed carefully. Empathy is a vital component of problem-solving and it is a human-centered design process. It also allows the design thinkers so they can make their assumptions and real insight is provided to the users. The second stage is to define the needs and problems of users. It allows the design team to collect great ideas to solve the problem. The next stage is to ideate when the challenges and assumptions are created (Neale et al., 2022).

This Stage Examines the Problem from Various Perspectives and Proposes Innovative Solutions

The fourth stage is a prototype, in this stage, a solution is created. The design team will create an inexpensive and scaled-down prototype. It will help investigate the key solutions generated in the ideation phase. The experimentation phase is important, it aims to investigate the best solution for chronic conditions which is identified in the first initial stages. The last stage is the test stage, in this stage evaluators and the designers will test the complete product by using the best solution which is identified in the Prototype stage. It is the final stage of all the stages. The major goal of this stage is to develop a deep understanding of the product and increase the number of users wherever possible (Vainauskienė & Vaitkienė, 2022).

Person-centered approach is adopted because, it helps the healthcare worker to understand all the problem, since all the problems are understood the care plan is designed accordingly to meet the needs of the people. Person-centered approach ensure that extra care and attention is been provided to the patient, so health and well-being can be ensured.

Analyse and Evaluate Sustainable Models for Using Self-management Support

There are several ways to analyze and evaluate self-management support, these are described below. The survey is an important method that is utilized and evaluated, there are some systems and services which are used so they can develop their survey. Some people use or adapt validated surveys or scales designed and tested by others, one needs to take care of adapting existing tools because any scales will have been validated as reliable. Self-management support is the way that is offering people an efficient method to deal with their chronic condition and facilitate daily life activities. Self-management support can provide encouragement and inspire people to get engaged more about their conditions. In addition, it is making the promotion of an active role in their health care. There are 7 self-management skills and are management of time, self-motivation, adaptability, stress management, goal alignment, decision-making, and personal development (Araújo-Soares et al., 2019). All these management skills should be present, these management skills ensure that every aspect of care is been provided and along with effective care is present. All the work which is carried out should be done in a well-defined time and that time should not be exceeded. Other than self-motivation is highly important, the employees should be motivated so they will return to work. Adaptability is another important characteristic, it allows the people to get adapted to any environment and work efficiently into it. Goal alignment should be present, in this the person should be aligned towards their goal and all the efforts should be put in that direction. Furthermore, the advantage of a self-management approach will be seen in terms of adopting a healthy lifestyle, and having access and confidence to use the support services. It will leads to the elimination of risk factors and promote a healthy nature.  The way self-management approaches will add on sustainable practices and give the range of support is commendable by which the long-term aspects can be managed.
